This page will help you understand why you are answering the Production Company Intake Form, how to navigate it, and how to answer the questions in the form. Before reaching out to an Amazon MGM Studios employee, please read through this guide as it may answer a lot of the questions you may have about the form.

Why am I answering this form?
Amazon MGM Studios keeps a record of all the Production Companies we might hire for production services to better understand their company infrastructure. The form will ask you basic questions about your company’s in-house teams and management, your company’s experience, as well as a few specific questions regarding your production and production finance resources. We recommend this form be answered by an executive at the company that has this knowledge about the company. Please note that the questions in this form are about your company as a whole, and not necessarily for a specific project.
How do I navigate the form?
You can access the form by clicking the button at the top of this page. Once you begin answering the form, it will autosave your responses in your browser for up to 15 days or until you clear your cookies/cache. This means you can close the browser and return to it within 15 days and keep your responses saved.

What is the company's website? | Enter the company's website if it has one. This will give us some insight into your company's previous work. This question is optional. |
What is your position within the company? | Please list what team you work in and your title at the company. This will help us understand if you are the correct person to reach out to if we have any follow-up questions, or if we should reach out to the head of a specific department. |
What is the company's address? | If your company has multiple locations with different addresses, please list the address of the office/branch/entity where you work out of. |
What year was the company founded/ incorporated? | If your company has multiple locations, please list the country where the branch/ office/ entity you sit in was founded/incorporated in. |
What year was the company founded/ incorporated? | If your company has multiple locations, please list the year in which the branch/ office/ entity you sit in was founded/incorporated. |
What is the company's Tax ID Number? | The term used here will vary based on the language you select. This number refers to the number that federal fiscal authorities use for taxation purposes. |
Please list the company's key company management (e.g., CEO, director) and their respective titles. |
Use this field to list the names and roles of the key managers at your company, this may include CEO, CFO, COO, and other associated titles/roles. This is specifically for key management position, and not every head of a department at your company. For example, if you have an in-house legal department, you do not need to list their name here, since there is another field for you to input the names of heads of specific departments. |
What is the size of the company, by approximate number of employees? | If your office has multiple locations, please list the rough estimate number of employees that sit in your branch/ office/ entity. |
Please select all the departments at the company that have in-house, full time staff. |
Please select the teams that your company has a hired full time staff for (i.e., if the team sits in your office space and is on the company's payroll). Please note that your company is not expected to have all of these in-house teams. |
Does the company have an outside legal counsel they work with for day-to-day production matters on a regular basis? | Select yes to this question if you have an outside legal counsel that you work with on a regular basis. |
Has the company fully delivered projects to other streaming services? | This includes both local and global streaming services. It will help us understand if your company is familiar with streaming services workflows. |
Similarly, how many projects does the company typically have in production/post at any given time? | This question will help us understand how many projects the company typically has in production/post, so we can better understand the company's bandwidth. |
Please select all Health and Safety policies implemented on projects the company produces. | This question will help us understand if your company is familiar with these policies and has implemented them in other productions. Our Health & Safety team will help your company understand the policies and requirements closer to production. |
Please select all trainings that the company typically provides to the cast & crew. | These are trainings that are typically provided to the cast & crew of a production during or before physical production commences. Our Production Operations team will help your company understand the trainings and requirements closer to production. This question will help us understand if your company is familiar with these trainings and has implemented them in other productions. |
Does the company work with 3rd party vendors for environmental and structural concerns when securing locations so that health and safety measures are met? | This question will let us know if you have local vendors you use to ensure locations are safe for physical production. There may be different requirements for confirming a location's environmental/structural safety per country. |
Has the company been investigated by local health and safety entities for issues or concerns regarding safety compliance? | This question does not include routine inspections. Only select yes to this question if there have been investigations conducted for specific health & safety concerns in the past. |
Has the company received any of the following from a statutory regulatory body? | Statutory Regulatory bodies vary based on your country. The options will be listed in this question, select any that are applicable or select “N/A” if none apply. |
Please provide a list of all types of insurance the company maintains, include coverage, limits, deductibles, effective dates and carriers for each. | Amazon MGM Studios will ask that your company uses the insurances provided by us closer to physical production, but it is helpful for us to know if your company maintains any insurance. |
If a project requires more than one currency, can the company set up bank accounts to allow funding in those currencies? | Some productions use locations in different countries or hire cast/crew from different countries that may require multiple currencies, so it is good for us to know if your company has experience paying in multiple currencies. |
If the company is contributing financially to a project, will a bank loan be required? | This question is applicable in projects where your company may be paying for part of the production. If that is not the case, please select "no". |
Who is the company's preferred payroll vendor? | This question will help us understand if you have a local vendor you regularly use for payroll. This question may not be applicable in your territory, if that is the case it will not show up as part of the questionnaire. |
Has the company applied for film tax incentive programs? | Amazon MGM Studios will help with tax incentives, but it is helpful for us to know if your company has experience in applying for them. |
Which states and/or countries is the company familiar in applying for tax incentives in? | This question will inform us of your company's experience applying for tax incentives in different states/countries. |
What software does the company use for creating budgets? | This question will help us determine how to best work with your company for the creation of budgets for a title. |
How experienced is the company in creating budgets using Movie Magic Budgeting? | You will only see this question if you select "Movie Magic Budgeting" in the question above. This question will help us understand the level of experience/familiarity your company has with Movie Magic Budgeting software, and if any assistance from our teams will be required to create these budgets. |
Select the appropriate descriptions of the production infrastructure the company has. | It is not required that the company has all of the production infrastructure elements listed, but it is helpful for us to know what facilities the company has. |
Does the company have multiple production entities? If so, please list what states/countries they are located in. | It is not required that the company has multiple production entities, but it is helpful for us to know this information to know what countries your company can provide production services. |
Is the company owned by a larger organization or group? If so, which one? | Your company may not be owned by a larger organization or group. |
What territory(ies)/ country(ies) does the company plan to provide services for Amazon MGM Studios in? | Please only select the territories/ countries where the company plans to provide production services to Amazon MGM Studios. This question will help us determine which of the questions below you should answer. There may be information we would like to know depending on where your company plans to provide services to Amazon MGM Studio. |

What accounting systems does the company use for production cost reporting and accounting? | The systems listed here vary based on your country; your company is not expected to use or be familiar with all of the systems listed, but please select whichever ones (if any) your company uses. |
Does the company use P.O.’s in your production accounting workflow? | The term P.O. may vary based on your country. A P.O. is the commercial document issued by a buyer to a seller, indicating types, quantities, and agreed prices for products or services required. |
Does the company have experience in establishing Single Purpose Entities (SPEs) for use in productions? | This question is only applicable if your company will be providing services to Amazon MGM Studios in the United States. |
Is the company experienced in complying with commercial motor vehicle and driver requirements (ex. FMCSA/DOT)? | This question is only applicable if your company will be providing services to Amazon MGM Studios in the United States. |
Has the company successfully received VAT refund on past projects? | This question is only applicable if your company will be providing services to Amazon MGM Studios in LATAM. |
